What is CalAIM?

CalAIM (California Advancing and Innovating Medi-Cal) is a multi-year initiative by the California Department of Health Care Services (DHCS) to improve the quality of life and health outcomes of Medi-Cal members through broad delivery system, program and payment reform across the Medi-Cal program.

This resource page has been developed to provide tools and resources to help providers easily navigate the CalAIM program so they can better serve our members. On this page you will find the most current information – guides, forms, trainings and more – as well as the latest updates from our Plan. Please use the Enhanced Care Management (ECM) Program Completion Questionnaire below to:

  1. Complete required 6-month reassessment of member needs for ECM services.
  2. Assess member readiness to graduate from ECM program, as appropriate.

Based on the results after assessment:

  • Members recommended to graduate from ECM program for lower level of care – If the recommendation is that the member is ready to move to a lower level of care, please maintain the completed assessment with the member’s file for audit purposes. You do not need to submit all assessments to California Health & Wellness Plan.
  • Members recommended to continue ECM services – If the recommendation is that the member continues ECM services, submit the completed assessment with your request for a new authorization. Please ensure that the start date on your request is set after the expiration date of the existing authorization.
