22-207m Improve Care for Members Who Need Skilled Nursing Services with Help from Rockport

Access Rockport facilities and case management services to ensure comprehensive care for members

Starting May 1, 2022, all Rockport skilled nursing facilities are transitioning from the patient driven payment model (PDPM) to episodic payment model (EPM). EPM will assist by:

  • Reducing the length of stay.
  • Ongoing engagement care coordination.
  • Increasing quality of care so the member can go back to the community and not be re-admitted back to the hospital.
  • Decreasing out-of-pocket expense for the member.

Long-term care is not applicable to Rockport EPM.

Rockport skilled nursing facilities

Discharge planners for our shared risk participating physician groups and hospitals are encouraged to place SNF members needing SNF admissions to these facilities.

Rockport services are available 24 hours a day, seven days a week at 800-949-2159, option 4.

  • Business office hours are Monday – Friday 8 a.m. to 8 p.m.
  • An on-call system is available before and after business hours, including weekends and holidays (same telephone number).
