California Health & Wellness Plan (CHWP) wants to provide the best care we can to our members. That means it’s important for us to know if you plan to move, change telephone numbers or leave the network. You can update or verify your contact information or status anytime by:
· Completing the Provider Data Reporting and Validation Form online at www.cahealthwellness.com/providers/resources/provider-data-reporting.html.
· Emailing CHWP at CAProvData@cahealthwellness.com.
· Calling Provider Services at 1-877-658-0305.
You can also check your information on our secure provider portal at www.cahealthwellness.com.
Please let CHWP know at least 30 days before you expect a change to your information.
LexisNexis® Outreach
If you are directly contracting with CHWP, you may receive biannual requests from LexisNexis® Risk Solutions to verify your information by the following methods:
· Email (primary method)
· Fax
· Telephone
It’s important you provide timely responses to the required biannual outreach requests to confirm that your information is correct as is or needs corrections. Practitioners that are nonresponsive to these requests are required to be removed from the plan’s directories, and could also experience a delay of payment or reimbursement of a claim.
