Makena added to Pharmacy Benefit

California Health & Wellness provides intramuscular FDA-approved Makena® (hydroxyprogesterone caproate injection) for eligible women. Makena® is used to lower the risk of premature birth for women who have previously had a premature baby. Members must meet the following criteria:

• History of preterm delivery at less than 37 weeks of gestation
• Current singleton pregnancy
• Therapy will begin between 16 weeks, 0 days and 27 weeks, six days of gestation

Members who meet the criteria for Makena® will receive up to a total of 21 doses to reach week 37 (through 36 weeks, six days) of gestation or delivery, whichever occurs first.

Please fax prior authorization (PA) requests for Makena® to our Pharmacy department at 877-259-6961. For approved Makena® requests, please contact AcariaHealth at 1-855-535-1815 to provide a prescription and arrange delivery.
