19-205 Weight Management Coaching Program Ending in April 2019

Members can access weight control resources through the Health Education Department

As of April 1, 2019, California Health & Wellness Plan (CHWP) is not offering the weight management Coaching program. Progress notes and the provider referral form will no longer be needed or accepted. Current members will be given the chance to take part in other wellness programs based on eligibility.

Weight control resources will still be made available. Interested members may contact the Health Education Department at 1-800-804-6074.

Providers are encouraged to access CHWP’s provider portal online at www.CAHealthWellness.com for real-time information, including eligibility verification, claims status, prior authorization status, plan summaries, and more.
